More On "Checklists"

Did you know that the John Hopkins School of Medicine established the Center for Innovation in Quality Patient Care, a learning laboratory that teaches front-line care providers how to troubleshoot and helps them get the resources to do it?

For example, on many units, the use of “checklists”, like pilots use before take-off, has slashed the incidence of catheter-related bloodstream infections by more than 50 percent. And, the “checklist” concept also has been used in the Hospital’s intensive care units to improve patient care rounds.

Sounds like customizable e Podiatry Consent Forms, doesn’t it.
Just too new-wavey, you say?
Nope, Hopkins began the "checklist" program in 2004!

The actual e Podiatry Consent Forms™ software program-file is divided into more than 50 "live consent form templates" for each version.

But, here we give you two sample templates to evaluate and review our style, format and extreme customization capabilities. Simply scroll up/down, or use arrow keys, a pointer or a mouse to locate the consent form sample template that you need.

Select the appropriate "blinking cursor" procedural form. Complete the patient and practitioner information prompts by customizing the "live" yellow-cue document highlights; and then print, save and repeat as often as you like.

A Product Review

It takes about 5 minutes to learn how to use the e Podiatry Consent Forms™ program. If you are already familiar with Microsoft WORD®, it takes about 2 minutes, or less. Just realize the yellow highlights are “live and customizable”, and ready for patient and procedural modification; all else is fixed.

Then, save, print and repeat as much as you want; just like the instructions say. Don’t worry; any character symbols do not translate to the printed consent form pages, which always look perfect.

Feel free to use B/W or color ink; or change fonts, style and size. Place you practice name, address and/or logo on each form, as well. Do whatever you wish!

This is a really a slick, and simple, program. But, my suggestion is to play with the website samples before you purchase. Or, let your secretary do so. Then, buy and use with confidence!

Extreme Customization Available

Did you know that e Podiatry Consent Forms™ is deeply customizable? Well, it is!

I was a beta-tester for this product and was very pleased at its ease of customization. Simply type in your changes, deletions or updates etc; then print and save for future use. As your surgical expertise grows, and patients and venues change, so will your consent form modifications? Start off with Version I, and progress to Version II, in due time.

Or, when satisfied with your modifications, use the same form for each surgical procedure by replacing your patient’s name, as needed. There is no time-bomb or license-limit. You can even create a new-original form, for each and every patient, for each and every procedure, and for each and every surgery.

With e Podiatry Consent Forms™, you are in complete control of the surgical consent process; from [1] informed patient discussion, to [2] document preparation, and [3] form redaction.

Make extra hardcopies too; for patient charts, ASCs or hospital use. Or, electronically store in your digital files. Yes, it’s that easy. MSFT WORD® 2003, or higher, is required.

Customize > Print > Save > Repeat
